When challenged by the Erie St. Clair Local Health Integration Network (LHIN) to analyze, evaluate and present a complete supply chain plan for home and community care services, TransForm, a Shared Service Organization (SSO) created and implemented the first successful integration of home and community care supply chain processes with an existing hospital-focused SSO in Ontario.

Their transformational model provided end-to-end automation, inventory controls and supply chain best practices which eliminated redundancy and produced significant cost savings. This was achieved through contract management optimization, implementation of supply management controls, elimination of manual processes and implementation of accurate patient case costing.

The efficiencies gained have proven to have significant impact on Provincial initiatives to drive better delivery of patient care while re-investing over a million dollars annually in cost savings back into direct patient care.
